• Customer data is not shared with 3rd parties for promotional or marketing purposes.
• Mobile opt-in and consent are never shared with anyone for any purpose. Any information sharing that may be mentioned elsewhere in this policy excludes mobile opt-in data.
Elite living services Messaging Terms and Conditions
1. The messaging program consists of general conversational messaging to answer questions and provide support to customers, undefined, promotional offers or discounts, any promotion of your products/services.
2. You can cancel the SMS service at any time. Just text 'STOP' to the phone number from which you received messages. After you send the SMS message 'STOP' to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.
3. If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly at gbaldwin@elitelivingservices.org.
4. Carriers are not liable for delayed or undelivered messages.
5. As always, message and data rates may apply for any messages sent to you from us and to us from you. Message frequency will vary based on communication needs. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

  • Health and Wellness Information (Non-Medical): Information related to daily living activities, physical limitations, dietary needs, personal preferences, schedules, and specific care instructions necessary for service delivery (e.g., reminders for medication schedules, mobility assistance requirements). Note: ELS does not collect or store Protected Health Information (PHI) as defined under HIPAA, as we provide non-medical services only.

  • Financial and Billing Information: Payment information, insurance details (for verification of non-medical benefits, if applicable), and billing history.

 

C. Technical and Usage Data

  • If you use our website or client/family portal (if applicable): IP address, browser type, operating system, usage patterns, and access times.

  • Directly from You: When you inquire about services, complete initial assessment forms, sign service agreements, or communicate with our staff (via phone, email, or in-person).

  • From Family Members/Authorized Representatives: When an authorized family member or representative provides information necessary for service coordination.

  • From Caregivers/Employees: Information collected during the hiring and employment process, and service documentation created during service delivery.

  • Automated Means: Through the use of cookies or tracking technologies on our website (if applicable).

  • Service Delivery: To assess client needs, develop personalized care plans for non-medical support, coordinate services, schedule caregivers, and manage the quality of in-home support.
